This standard claims form should be used for the lodging of claims in accordance with Chapter IV of Regulation (EU) 2015/848 on insolvency proceedings. Claims for exclusion of assets from the debtor's assets should be filed in accordance with national law.
The use of this standard claim form for filing claims is optional. However, when filing claims by means other than this standard form, the claim must contain all the information specified as mandatory in this form.
